First Visit

Brief Paperwork

At Active Chiropractic & Rehabilitation, we understand that your time is valuable, so we offer you the opportunity to print and fill out our paperwork at your convenience. If you're unable to do this, we recommend you arrive for your appointment 10-15 minutes early to allow time to complete the paperwork before your scheduled visit.

New Patient Examination

All new patients beginning treatment with us receive a new patient examination with the doctor to discuss your health-related problems, concerns, as well as potential treatment options. The doctor will ask you various questions related to your condition, and perform a series of specialized tests to determine which tissues are affected and to what extent, and to locate the source of the problem.

Please remember that if your affected area will be obstructed by clothing (such as tight jeans over a knee injury), we recommend coming prepared for the exam wearing loose clothing, or bringing a pair of shorts or t-shirt to change into here.


Your specific condition may require the use of x-rays or other imaging to either rule out more serious conditions or assist us in developing the most effective treatment plan for you. If this is the case we will advise you on when and where to receive these services.

Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, physical therapies, and/or soft tissue therapies.

Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.

Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.
